There is an extra loop at the top of the shoes just next to the last eyelets. This loop is connected to the heel of the shoe that allows for a running shoe fit.Footnotes: Please remember you should not normally want to fit your diabetic patients either shorter or narrower than measured. However, by following these instructions, you will achieve a fit comparable to the other shoes in our line. It is also important to remember that when you are fitting patients that have narrow heels, it will be easier to accommodate their needs with a lace shoe.

I did not expect the shoes to be so comfortable. I love them. I have a wide foot and this shoe gives me plenty of room. I also have Rheumatoid Arthritis and need my shoes to feel great from the heel through the toes. So far this shoe have met all of my expectations. A big plus is that they look really cute! I thought I would get something that looked like a bowling shoe! That is not the case, this shoe is very fashionable for an athletic shoe.I have other "extra depth" shoes, and the extra depth comes from the fact that the shoe has multiple insoles, and you can take out the extras to create the right fit for you. This shoe actually comes with NO insoles inside. The company provides 2 you can choose from, but once you put one or the other in, they take up all the extra space. Additionally, my foot measures at a 6.5, but I tend to buy a larger size in hopes of getting the width right. I ordered a 7.5 and the shoe was rubbing my toes on both sides.I was looking for a shoe to use after my hip surgery and needed "elastic" shoe laces which was what I specified. The shoe was also much higher on the top arch of the foot which made it feel suffocating. The toe bed was more narrow that is recommended for a good fit because it still has the capacity to pinch. The set of alternative liners made the shoe dramatically smaller and left me feeling like my foot was either too tight, or unsupported. I ended up going with a much lower-end brand of shoe that had light weight and more forgiving structures. These are amazingly comfortable. I stand on concrete 8-10 hours a day and have been trying to find a shoe that doesn't leave my feet hurting at the end of the day. Five pairs of shoes later and lots of wasted money I finally found the right shoes for the job.Meghan Diabetic Athletic shoes fit as expected and are light as air. They came with two types of inserts for arch support and a long-handled shoe horn (remember those?). Great shoes. No big deal! But its o.k., I definitely had better!Ran somewhat small. My arches hurt if I wear them longer than a couple hours.a little tight
